The Coalition is a grass-roots organization made up of volunteers committed to raising public awareness of the issues at the end of life. They believe that knowing more about the options for end-of-life care, whether it be in a hospital, nursing facility or at home, will improve the quality of living, and better prepare the person for this important stage of life.

An integral part of the Coalition program is educating the community about the importance of advance care planning and completing an Advance Directive for Health Care. The Make Your Wishes Known Initiative is dedicated to this end. [More…]
